Three Types of Placards

Here is an overview of the three types of Placards available

Short Term

Short term placards last up to 12 months. If you are suffering from short-term disabilities, you may be able to apply.

If you want to apply, you must speak to your doctor about a disabled placard application. Once your doctor completes the application, you can then come into our office and speak to one of our staff members who will assist you then.

Long Term

Long Term placards run for up to 5 years. Once that term has expired, you will have to reapply to renew it.

Like the short-term placard (the red one), you will have to speak to your doctor to complete an application form for a long term disability. Upon completion of the disabled placard, you can then bring the form into our office and receive your long term placard.

Permanent

Like the long-term placard, the permanent placard will last up to 5 years, which at that time, you will have to apply to renew it.

To renew your placard, you have to complete the self-declaration section of the form and give it to on of our staff members who will then assist you accordingly.
