Legal Name Changes
If you are looking to have your name changed, you will have to sign and submit a form to Vital Statistics.
You may also have to submit fingerprints with the application.
Requirements
To apply for a name change, you must:
- be 18 years and older
- Be an Albertan resident
- Apply to change one or more of the following:
- Their own name
- Current Spouse’s name
- Your child’s name
- Not apply changes to:
- Your spouse’s child’s name when you are not the dependant by birth or adoption,
- or, if you do not have custody
Steps to Change Your name
- Step 1: Office Visit
- Visit our office with personal identification and complete an application form
- Step 2: Fingerprints & Supporting Identification
- Bring your fingerprint form and:
- Proof of marriage
- Proof of divorce
- Proof of custody
- Proof of guardianship
- Proof of death of a spouse/partner/parent
- Proof of legal name/proof of birth
- Proof of annulled marriage
- Court order with consent
- Step 3: Review your name change certificate
Once all of the aforementioned steps are completed, you should receive your certificate within 4 – 6 weeks.
